Register With The Practice?

When registering with the practice, the registration form is completed by the applicant together with a patient questionnaire. The applicant will be invited for a routine screening check to enable the practice to compile all the information in the records.

The patient has a right to express a preference to receive services from a particular doctor or nurse either generally or in relation to any particular condition. The practice will endeavour to comply wherever possible (the practice will record in writing any such preference expressed by or on behalf of the patient).
